CIRCLE OF CARE TEAM
The Old Colony Hospice Circle of Care Team
includes:
The patient's physician, an integral part of the team who
oversees the plan of care;
The hospice Medical Director who oversees the medical services provided by
the team;
Registered Nurses specializing in pain management and controlling
symptoms;
Licensed Social Workers assisting the patient and family with emotional
support;
Hospice Aides helping the patient with personal care needs;
Pastoral Counselors serving people of all faiths (or no faith) and
outreach to clergy;
Bereavement Counselors providing support groups, and one-to-one counseling
during the year following the death of the patient;
Trained Volunteers who visit, help with grocery shopping/errands, and
provide companionship.
The team does not move into your home, but will provide
these services as needed and consistent with the Plan of Care devised by
your Primary Nurse.
